What Bone Density Scans Compare You To
Bone density results are expressed as scores relative to a young adult reference or an age-matched one, and which comparison is used changes what the result means.

Bone density results are reported as scores rather than as raw measurements. The scores are comparisons, and knowing what they compare against is necessary to interpret them.
The scan measures attenuation, not bone
The standard technique passes low-dose X-rays at two energy levels through a region and infers mineral content from how much is absorbed.
The result is expressed as mineral content per unit of projected area, which is an areal rather than a volumetric density.
Because it is areal, larger bones produce higher readings for the same true density, which is one source of systematic variation between people.
Two reference comparisons are used
One score compares the measurement against the average peak density of healthy young adults, expressed in standard deviations from that average.
The other compares against people of the same age and sex, indicating whether the result is typical for that group.
These answer different questions. The first asks how much has been lost relative to peak; the second asks whether the loss is unusual for the person's age.
Diagnostic thresholds use the young-adult comparison
Formal classification of low bone density is defined by cut-points on the young-adult score, chosen decades ago to identify a prevalence consistent with observed fracture rates.
The age-matched score is not used for diagnosis, because loss that is average for an age group can still carry substantial fracture risk.
This is why a result described as normal for age may still fall into a diagnostic category on the other score.
Density is one input to fracture risk
Most fractures occur in people who do not meet the diagnostic threshold, because there are far more of them than of people who do.
Risk calculators therefore combine density with age, previous fracture, family history, medication use and other factors to produce a fracture probability.
Density alone describes one property of the bone and says nothing about falls, which are the immediate cause of most fractures.
Comparing scans requires care
Repeat scans are meaningfully compared only when performed on the same machine at the same site, since equipment and region differ systematically.
Change also has to exceed the machine's measurement precision before it means anything, which is why repeat intervals are usually measured in years.
What any result implies for an individual is a matter for the clinician who ordered it, since the same score carries different weight depending on everything else in the history.
