What A False Positive Costs A Screening Programme
Incorrect positive results are not merely inconvenient but drive follow-up investigation, anxiety and occasional harm, and their frequency rises as more people are tested.

Discussion of screening tests usually focuses on whether they detect disease. The frequency of incorrect positives is equally consequential and is what limits how widely many tests can be deployed.
Two error rates are always present
Sensitivity describes how reliably a test identifies people who have the condition. Specificity describes how reliably it clears people who do not.
Both are properties of the test, and they trade against each other. Adjusting a threshold to catch more true cases inevitably flags more people who are not affected.
No test escapes this. Where the threshold sits is a decision about which kind of error is more acceptable, given what happens after each result.
Rarity multiplies the false positives
Even a highly specific test generates a fixed small proportion of incorrect positives among those tested. In a large population, a small proportion is a large number of people.
If the condition itself is rare, that number can exceed the number of true cases substantially. The majority of people receiving a positive result then do not have the condition.
This is counterintuitive, and it is the single most common misunderstanding about test results, including among people who use tests professionally.
The cost is downstream, not in the test
A positive screening result initiates further investigation, which may involve imaging, biopsy or specialist referral, each with its own burden and occasionally its own risk.
The period between the initial result and its resolution carries substantial anxiety, and studies of screening programmes find that this can persist after the all-clear.
Some people also change their behaviour or their engagement with healthcare afterwards, in both directions, which is a further consequence the test result itself does not capture.
Repeated screening accumulates the risk
A per-round false positive rate compounds across a lifetime of invitations. The chance of experiencing at least one incorrect positive across many rounds is far higher than the rate for any single round.
Programmes therefore consider cumulative rather than single-round figures when deciding intervals and thresholds, since the lifetime experience is what a participant actually faces.
Why this shapes what is offered
A test can work well and still be unsuitable for population screening if the follow-up it generates is invasive or the condition is rare enough to swamp true cases.
Direct-to-consumer tests sit outside this calculation entirely, since they are chosen by individuals rather than deployed by a programme that has weighed the trade-off.
Anyone receiving an unexpected positive result from any test should discuss it with a doctor before acting on it, because interpretation depends on their particular risk.
