Standing, fidgeting and the movement that never gets counted
A large share of daily energy expenditure comes from activity nobody would describe as exercise. Its variability between people is larger than most assume.

The activity that goes unrecorded
A substantial part of what the body does each day consists of shifting position, walking between rooms, standing while talking and the small movements people make without noticing. None of this appears in any exercise log, and it is largely invisible to the person doing it, which is why it is systematically left out of self-assessment. The total varies considerably between individuals with similar occupations, and the differences are large enough to matter over the course of a week.
Some of that variation appears to be constitutional, with certain people simply moving more in an unplanned way than others do. The rest reflects environment, since a workplace requiring movement between floors produces a different daily total than one where everything is within reach.
Why sitting became a separate question
Time spent sitting is not simply the inverse of time spent exercising, since a person can complete a structured session and still spend most of the remaining day seated. This is why the two are examined separately, and why the association between prolonged sitting and health outcomes is not fully explained by activity levels. Whether prolonged sitting causes harm independently, or reflects other differences between people who sit more and people who sit less, is not fully resolved.
The popular framing of sitting as equivalent to smoking is an overstatement that has been criticised repeatedly and continues to circulate anyway. The more defensible position is that long uninterrupted periods of sitting are worth breaking up, without claiming to know exactly how much that helps.
What breaking it up appears to do
Muscle activity in the legs during standing and walking affects how the body handles nutrients circulating after a meal, which is a plausible mechanism for short breaks. The effects observed in short-term measurements are real, and translating them into long-term health outcomes involves a step the evidence has not fully taken. Frequency appears to matter more than duration, meaning several brief interruptions differ from one long break of the same total length.
This makes the practical version unusually low-effort, since it requires rearranging when movement happens rather than adding anything new. It also fits the pattern of most workplaces better than advice requiring dedicated time that few people control.
Standing desks and their limits
Standing to work does increase energy expenditure slightly compared with sitting, though the difference is smaller than the enthusiasm for these desks implies. The more plausible benefit is that standing makes small movements easier and more likely, rather than the standing itself doing the work. Standing for long unbroken periods brings its own discomforts, particularly for the legs and lower back, and is not simply an improvement on sitting.
Alternating between the two appears more sensible than either extreme, which is the conclusion most reviews of this area arrive at. Anyone with existing back, hip or circulatory problems should take advice specific to their situation rather than following a general recommendation.
Keeping the claim proportionate
Incidental movement is worth taking seriously because it is large in total and requires no motivation, not because it substitutes for anything else. It does not appear to produce the specific adaptations that come from more demanding activity, and the two are complementary rather than interchangeable. Counting it accurately is difficult, which is one reason device step counts feel motivating while being only loosely related to total expenditure.
The useful reframing is that the day is already full of movement opportunities that cost nothing and are frequently designed out of modern buildings. Where mobility is limited by pain or a medical condition, what is appropriate should be worked out with a physiotherapist or doctor.
