Slow breathing is recommended so widely that the mechanism is rarely stated. There is a reasonably well-described physiological pathway, and it explains why particular patterns are specified.

Breathing is unusual in being both automatic and voluntary

Most autonomic functions cannot be directly controlled. Heart rate, digestion and blood vessel tone are not accessible to deliberate instruction.

Breathing is regulated automatically but can also be driven voluntarily, which makes it the one point at which conscious control reaches the autonomic system.

Interventions that aim to influence autonomic state therefore work through breathing by default, not by preference but because the other routes are unavailable.

Heart rate varies with the breath

Heart rate rises slightly during inhalation and falls during exhalation. This variation is mediated by the vagus nerve, which exerts a braking influence on the heart.

Vagal influence is reduced during inhalation and restored during exhalation, producing the rhythmic fluctuation observed in any reasonably sensitive heart rate recording.

Because the effect is tied to the exhalation phase, patterns with a longer exhalation than inhalation produce a greater average vagal influence than even breathing does.

A resonance frequency exists

Pressure sensors in the major arteries feed into a loop regulating blood pressure, and that loop has a characteristic delay which gives it a natural oscillation rate.

Breathing at approximately that rate, which for most adults falls around six breaths per minute, brings the respiratory and blood pressure rhythms into phase and amplifies both.

This is the basis of the frequently cited slow-breathing rates. They are not arbitrary; they approximate the frequency at which the two oscillations reinforce each other.

Depth is not the active ingredient

Instructions emphasising very deep breaths can produce over-breathing, which lowers carbon dioxide levels and causes dizziness and tingling.

The mechanism depends on rate and on the relative length of the phases rather than on volume, so a slow, unforced breath achieves it better than a large one.

What the effect does and does not cover

Measurable changes in heart rate variability and blood pressure occur within minutes, and these are among the more consistently replicated findings in the area.

Whether that translates into sustained change depends on repeated practice, and the effects on a stressful situation itself are obviously nil.

People with respiratory or cardiac conditions, or with panic symptoms that breathing exercises can worsen, should take guidance from a clinician rather than a general protocol.
